This Application Form is for the *Commercial Funeral Trade Only. (ie. Funeral Homes, Crematoriums, Funeral Directors etc)

*YOUR BUSINESS MUST MEET THESE BASIC REQUIREMENTS TO APPLY:

• Applicable State or local business licenses
• Federal Identification Number or Resale Tax certificate
• The day-to-day Business operations must be predominately conducted in a structure assigned for the purposes of operating a business; Home-based businesses will not be considered.  

*Individuals without credentials such as a registered business or appropriate license’s for your state, will not be considered as part of our Commercial program and restricted from purchasing our Commercially Discounted products that have been categorized for funeral professionals only. These individuals will be classed as Consumers and can only purchase products at our Consumer Retail Prices.
